Wheelchair for a long period
Can you no longer move around in and outside your home by yourself due to an illness or disability? Do you need a wheelchair every day? Do you need the wheelchair for longer than 6 months? If so, you may be eligible for a wheelchair.

You can borrow a wheelchair from a home care shop.
Do you live in a care facility?
Do you live in a nursing home or disability care institution, for example? Are you not paying living expenses such as rent, gas, water, electricity for this? Apply for a wheelchair from the Zorgkantoor Friesland.
Do you need a wheelchair for work or education?
Do you need a wheelchair to do your job properly or go to school? Apply for a wheelchair from the UWV.
Wheelchair needed for other situations?
If so, you may be eligible for a wheelchair under the Social Support Act (Wmo). Contact the City of Leeuwarden to discuss this.
How do I submit an application?
Submit your application online (you will need DigiD). Are you unwilling or unable to apply online? Download and complete the Wmo Application Form and fill in. Send the form by post to the City of Leeuwarden. You can also hand it in at the Town Hall.
What happens next?
A local authority employee will call you within 6 weeks of receiving your application. The employee will first ask for more information about your situation. This can be done through a telephone interview or a meeting at your home. A solution is sought together with you. The local authority will let you know within 2 weeks of the interview whether you are eligible for a wheelchair. You pay a personal contribution for a facility under the Wmo.
If there are many applications at once, it may take a little longer. The local authority will inform you of this.
